It's hard to avoid cigarette smoke in Amsterdam, but doable if you don't mind being lonely.  Some restaurants have non-smoking sections, but you WILL be eating by yourself.  For lunch, try the museum cafes.  The Anne Frank House, the Amsterdam Historical Museum, and the Jewish Historical Museum (kosher dairy) cafes are all smoke-free, with lots of food choices.  Smoke-free restaurants include Warung Marlon (Surinamese), Wagamama (a hip Japanese noodle bar), and the Golden Temple (vegetarian).   The Hotel Fita is entirely smoke-free, so of course, the comprehensive breakfast included with your room is sans cigarettes.
